Document Set adds extra/duplicate Default Content outside of Doc Set "folder"

The good - When I add the Document Set to the library (list) using the New button, the Doc Set is created in the library, and the Default Content (A blank test document) appears nested inside the Doc Set "folder."

 

The bad - The problem is that the Default Content is also adding itself to the Library (list) *outside* of the Doc Set "folder."  So I have a doc set with the default content inside it, and then a second/duplicate instance of the same default content document in the library outside of the doc set.  Screen Shots attached.

 

I have added the same Doc Set to a different library on our site and it does not cause this problem with the duplication of the default content outside of the doc set.  I have opened up the Document Library Settings of both libraries and looked at every single setting attempting to determine why this problem happens in one library or not the other.  I do not see a single difference.

 

I cannot recreate this library where I need to use doc sets to start fresh because there is too much work invested in it and I don't have access to copy SP library templates.  (My IT Department can't figure out why my SP site doesn't allow template copying but other sites at our company do.)  So I'm left at your mercy to help me solve this problem.  I've Googled like crazy but cannot find an instance of this happening to anyone else, including searching in this forum.  

 

(I need to add doc sets because I have an elaborate Power Automate flow that I created for multiple serial Content Approval that's working perfectly, but I need to use the doc sets to approve groups of documents and create a second flow in Power Automate.  I do not code so I do everything by adding Actions in Power Automate.)
