Flow not working with Forms

I created a flow yesterday that seemed to work without testing but when I tested the form today there was one step that was not completing. So I deleted the flow and created a new one. The new flow though would not allow me to select the list of responses even though I had pulled over the list of responses as one of the steps. It also was giving me multiple errors requiring me to sign in multiple times. Now I cannot create the flow that I need because I can no longer pull the individual responses from the form in order to route those details to specific approvers. Can someone help me to fix this?
