O365 CLIENT UPGRADE PILOT USERS FEEDBACK

I am managing 365 client upgrade pilot at this moment. I need to send out a feedback form to the pilot users to respond after they have done the upgrade. Where can I find questions to add to the pilot user feedback form for their experiences in this pilot program?
