how do I get immersive reader to work?

the icon appears at the address bar that you wanna click on

You haven't mentioned which Edge insider channel you, if you are on Beta, you need to enable a flag called "Reading View" in edge://flags/

but if you are using Dev and Canary channels, it's enabled by default but still you can enable extra features of it in edge://flags/

I also agree with you that the Immersive reader or its old name, Reader view mode, should appear on all web pages and not only those that Edge chooses to.

'Tis true, not all web sites have it, but, when there it looks like the red circled icon or button.
