No heartbeat from session host and wvd-agent doesn't start on custom image

Don't know if you resolved this, but I just had exactly the same issue with a custom image and the new VMs would not join the hostpool.
I found by manually uninstalling Windows Virtual Desktop Agent, Windows Virtual Desktop Agent Bootloader and the Windows Virtual Desktop side-by-side stack. When you uninstall the Windows Virtual Desktop sbss with the default options it will close down the Remote Desktop services, and you will have to manually restart the machine in the Azure portal.
Finally remove the C:\Program Files\Microsoft RDInfra folder.

Your machine will then be back in the pool.
