I think this method has been superseded by the "AGDiag" tool that is hosted on GitHub at https://github.com/microsoft/agdiag.

 

AG Diag can take the input from several tools, one of which is TSS which you can get from http://aka.ms/getTSS and collect the SQL TSS using 

tss.cmd SDP:SQLBase

 The person running TSS should be both a SQL Server sysadmin and also a Windows administrator.
