Zohawtof this is a nice concept.
However the forms are created in a users form area (the form creator). For business continuity and ensuring the template doesn't fall over when someone leaves it should really be linked to a group form. I see the form can be moved in Microsoft Forms from the user to a group but it seems that breaks the update template and it won't load.
Tend to agree with Todd_E312 the proliferation of Task management tools is getting a bit extreme. Certainly would be good for this as a task with a due date to appear in Tasks too.
I understand that we are trying to make these sorts of tools easily accessible for base users. But without the centralized data, reporting this is the reason power users tend to use forms/powerapps with power automate and lists to centralize and make easier for management and reporting.
