Windows Updates gets reverted on Server 2019 with Citrix
Hi
I have a few Server 2019 on Citrix MCS.. When trying to update CU patches it goes to 25% and then restarts and then rollback "Could not apply changes..". It has happened with CU2021-08, 2021-09 and 2021-10. I have tried DISM, chkdsk, sfcfix and also going through permissions in registry but I cannot seem to find it. Also did a complete Windows Update reset.
Updates are distributed via SCCM, but installing the CU patches manually from WU gives the same error.
In CBS.LOG, it looks like poqexec has issues creating directory "Accessibility".
I have created that folder manually but still the same issue, somehow it seems corrupt, yet DISM cannot find any corruptions.
Anyone have any suggestions I would be grateful.

- dntechCopper ContributorHaving same exact issue. Citrix, windows 2019, poqexec.log and cbs logs shows a CreateDirectory error in the start menu though mine is for Administrative Tools rather than Accessibility. Tried renaming the existing folder, giving full permissions, nothing seems to get it past this point.

dntech of MAslin1425 did either of you get a resolution? I am seeing the same error message but for the System Tools folder. Checked permissions, deleted and recreated the folder but the update always fails.

CMcBRHS I did resolve my issue. Ended up being a fslogix rule I had set to hide a start menu folder that Windows Update was trying to change. I temporarily removed the rules and was able to update. I've started adding an exclusion to all rules so that if I'm on the master image machine they don't apply.
