To do Items Scrambled

I have been using Tasks in Outlook, in these tasks I have them grouped by company name, I have  attachments and comments inserted into a task, when I look at the tasks the next day, they are sometimes missing the attachment or the comment is not there, or the order is not right. for example all of the text moves to the top and the attachments move to the bottom, how can I stop this from re-occurring, thank you
