To-Do in Apple Watch desynced

I've been using this for several months and when it got problematic, I would offload the app from my iPhone via Settings -> General -> iPhone storage. Then reinstall it, open To Do on the iPhone, and open again on the Apple Watch. This worked well (had to do it weekly) until a few weeks ago and now it is very buggy and unusable on the Apple Watch. Sometimes it won't re-sync my lists. When it does, if I check them off on the Apple Watch, a few seconds later the Apple Watch shows the task un-checked, though the iPhone does have it marked complete.
The issues have gotten worse over the last few months, and I may have to abandon using this app unless this is fixed soon.
