Using Forms with Stream Videos - How to make form automatically appear

We have some training videos that will require the user to answer some questions after viewing.  I've worked through the process of adding a Form to the video.  However, when the video plays the form does not pop up at the designated time.  I only see a dot on the timeline where the form is, and then a button that appears that says "Open Video Form".  Is there a way to make the form automatically appear when it gets to that point on the timeline?
