MicrosoftKarteek_Bandi It is difficult to identify the problem without seeing how you created the CLU in the Health Bot.

Most of the times, the Endpoint field is not set properly, please make sure to use the correct template: "https://{your-language-resource-name}.cognitiveservices.azure.com".
