found the solution to my problem. The webpart wasn't linked to the sharepoint list. If you edit the webpart, check if there's a list associated, if not just link the appropriate list, save and publish.

I've just deployed it in my tenant and it seems it cannot save requests/incidents.
Anyone experiencing the same issue ?

I have exactly the same problem. In general save button doesn't work. Chrome console shows that there's JavaScript error behind. I reported this on project GitHub site. I hope that I will get any suggestion..
