I created a whole account just to post this reply! 

 

The "Three Amigo's" option you presented at the end is EXACTLY what I am trying to accomplish at our company. For pretty much the main reasons you stated. Front Door does the global portion, App Proxy adds 2FA, and then at the backend we have an App Gateway that provides load balancing to the application on it's dedicated ports. We are also going to be adding WAF at the App Gateway.
