Hi John,
The ability to apply mailbox permissions is limited to org admins because it would give the person with that level of permissions unrestricted access to anyones mailbox.
So if you were to give that right to a person working the helpdesk they would be able to grant themselves access to the CEOs mailbox, their managers mailbox, etc. You would not know the access had been granted unless you were doing some auditing or you were to look at the permissions on the mailbox for some reason.
So the ability to modify that type of permissions was restricted out of the box to Organization Admins.
