I hope support can help, as I'm not sure this is the best place to answer these kind of questions, but I'll repeat what I said before - we're not turning off basic auth for SMTP. If you want to keep using it, keep using it. To one of your other requirements - make an OAuth app require Authenticator every time it opens - I'm not sure that's possible, someone else here might know how to set it up, perhaps you can mess with token lifetimes or use Conditional Access to enforce it, I'm not sure. But if it's not possible then, well, not sure what to say. Your IMAP mailer in the Medical App - the app needs to be updated to work with OAuth, or it's going to break. If you want to keep connecting to a mailbox in Exchange Online anyway.
