Microsoft Form, not loading in Microsoft Stream video 'login.microsoftonline.com refused to connect.

Hello, 

I used a test student account to check that my students could access the Microsoft form question I had put at the end of a Stream video. But when it got to the section in the video it said, 

login.microsoftonline.com refused to connect.

and would not load. Can anyone suggest what is wrong?

The video is an upload. Had no problem viewing it. As a teacher and owner of the video I have bene able to answer the question and my response be collected.

I and another user are also getting the same problem, we can open forms with the direct link but not from a streams video, ti either does a spinning circle or occasionally displays login.microsoftonline.com refused to connect, are there any fixes for this other than using the direct link or is it a temporary outage?
