Form flagged when adding new text input questions

Hi all,

 

My form was flagged yesterday with the following message: "This form can't be distributed as it is asking for personal or sensitive information". When I deleted the two questions asking for email and phone number, it seems to be working again. However, whenever I tried to add a new question with text input, even without typing anything in the question field, my form was immediately flagged again.

 

Does anyone know how to fix this problem??? Many thanks!

@christychen take a copy of the form then add the new  questions to that new form and see if that solves it.

Thank you for your reply. If we make a copy, it will have a different QR code, right? The thing is, we have our QR code for the original form sent out, it would be a bit difficult to swap. Is there any other ways we can try?
