I do not really see the point in having a separate section called "Provide Additional Information" where I can only select from 2 options (Text field and Select). It would be nice to just have the option to include these fields.

 

If this is not possible then two things should be fixed for the custom fields:

- Add a required * to the form field

- Make the placeholder text editable

 

So this is what it currently looks like:

Yes I totally agree. I believe the placeholder text being editable is huge too. 
I had to un- select phone number from customer information and add it as a custom field because I needed to specify that they should input the number they wished to be called at for the appointment. I had to add this in parentheses to the Question title which doesn't look very professional.
