The best upgrade path would be to build a new 2016 server and migrate any roles or features from the old 2008 R2 server to the 2016 server. Let me know if you need any more information.

Best thing to do first is to test it in a lab environment. If you do not have a lab, maybe use Azure.

 

Make sure you document everything running on the 2008 r2 machine. including services and what users they are using, task schedules, etc... then test, test and more testing.

I agree the best option is install the OS from the beginning, do a backup of your sever statement.
