I have a handful of users who's user folder's name changed to Documents and the icon also changed. I was able to rename the folder back to the user name and seems to work okay. What would cause this? You can see in my screenshot a few users do not have the folder icon, those are the ones that I had to rename. Any suggestions? Thank you.

Is it possible the users in question have logged onto an old Windows 7/Vista machine? I seem to recall this was a "feature" back then with simply calling the folder "Documents".

Yes, actually the users on on some older Windows 7 machines. We are in the process of upgrading our entire office to Windows 10, so I will wait until after we are all upgraded to Windows 10 and see if the problem persists.
