Find an app for process managament

Hi, I am not sure if this forum is the right place, if not please let me know where you think I should direct my question.

 

We use excel to create a single page document where we identify a problem for improvment.

Please find below attached excel template we use.

 

I am trying to find if there is an app we can use in TEAMS to solve this. When we create lots of these susggestions for improvment we also identify tasks. And keeping track of these are important.

I have tried planner, but that does not fit well for our purpose.

It can also be a stand alone software if there is no app in teams that fit.

 

Any sugestions would be appriciated.
