I am not sure if "Disclaimer" is the correct verbiage.
What I was asked to do was as follows:
1. When a user logs in to SharePoint, create a form with a disclaimer on it that asks the user to accept or cancel.
2. Whatever the user requests will be collected and stores somewhere.
IE: user joe.smith clicks on Accept - a DB will record his email answer and say a 1 for accept and 2 for cancel
Does that make sense and is this at all possible?
