How is the number of unique permission scopes determined?

According to https://docs.microsoft.com/en-us/office365/servicedescriptions/sharepoint-online-service-description..., there is an imposed limit of "50,000 uniquely-permissioned items". We are working on a solution which will involve approximately 3,500 records being created each day, and each record will need to have permissions set so that only site owners, the creator and the creator's manager will have permission. The site owner will need full control, the creator read and the manager edit. 

 

So, will that result in 3,500 unique scopes (the number of items) or 10,500 (3,500 each for site owner, creator and manager)?

 

Also, if "Item Level Permissions" is enabled on the list, does that change anything about how the limit is calculated? Or does it simply handle the assignment of permissions that contribute to the calculation?
