Form submission not creating item in SharePoint

Hi,

I have created a Form and a powerautoate flow to read the form entry and then add it as a new item in a SharePoint list.

When I submit a form or test the flow I get a success message but the item is not created in the SharePoint list.

Any ideas?

@Phil_Hesketh you'd need to post a screenshot of your flow for us to have any idea what's happening.

@Phil_Hesketh sorry, I can't see anything wrong there.. So not sure what's happening. Did you refresh your leist with ctrl+r to make sure you were seeing the most recent version?
