build a left navigation, like the O365 Administration portal

Hey,

I like to build a left navigation, like the O365 Administration portal, on my SharePoint publishing sites (modern site collection). How can I do this with the SPFx extensions? And get save with changes form Microsoft on the site?

Can you please give me some ideas for the architecture?
