Create Hyperlink for an Excel UserForm in PowerPoint

I would like to create a hyperlink in PowerPoint that will open an Excel UserForm that has already been created. I intend to use this link to obtain data from individuals who are viewing the PowerPoint. What would be the best method or technique to complete this task?

In the Insert Object dialog box, select Create from file. Click Browse, and in the Browse dialog box, browse to and then select the Excel workbook that contains the information that you want to insert, and then click OK. In the Insert Object dialog box, select Link and then click OK.
