Hi,
I want to create a upload form, when user clicks Upload, it should get a Form, Has a Name, checkboxes, 2-3 textbox. and 2-3 upload boxes for documents.
In the second step it should automatically, create a folder that has the name of that upload form and contains the uploaded files.
Do you know is that possible?
