Connection error/fix prompt in SharePoint/Power App & Power Automate Integration

I developed a PTO tool using SharePoint List with Power Apps & Automate integration. And users are running into Connection errors when they think that they have allowed Power Apps and Power Automate to access their connections.

Does anyone know the reason behind the error? We would like to add the explanation into our tool-guide in FAQ section.

Probably password change, MFA change etc etc. It can be multiple reasons
