Returnable form in outlook

Is it possible to create an outgoing form in an outlook email that the receiver when replying can fill in the answers to questions, select multiple choice answers or yes/no check boxes?  If so where could I find more information on this?  Thank you.

@JLRRIA it's possible using custom forms in outlook... but only works with coworkers (using outlook) and the form needs published in the organizational forms library. 

 

A link to Microsoft forms would be better but is not 100% within email.
