Pop-up Survey after a meeting with a client

Is there any way of creating a pop-up survey - that comes up on your phone after a scheduled Outlook or MS Teams meeting has taken place that asks three simple questions:   
  1. Were any competitors mentioned at this meeting? Y/N - if yes who
  2. Were any business needs/gaps mentioned? y/n - if yes provide a short summary
  3. Was our company praised or disapproved in this meeting? y/n - if yes provide a short summary
