Power Automate with Dynamics Customer Voice

Hello all, I just moved to the new Forms Pro platform Dynamics 365 Customer Voice. I have created a survey and a flow for it to follow. Basically the flow goes as follows: Customer submits survey -> a new task is (supposed to be) added to Planner -> email sent to owner of the form. 

However, I keep getting an email saying my flow has failed. Not sure why as it doesn't give me any error codes or anything. I'm also new to creating flows so any advice would be helpful! Happy to send screen shots of everything as well.

para ayudarte te pido que agregues una captura de pantalla de la ejecución fallida, y otra captura de tu flujo.
