Creating form to collect quantity and reason why customer contacted our Servicedesk

Hi! I'm working with small service desk team and we would like to start measuring how many customer contacts our two Servicedesk agents each day. They are using email, ticketing system and phone but we cant get data from these systems to get an overview of last week activities.

 

Can I make data collection form with excel or some other tool with these specifications?

  • Visual form with simple line's ''email'', ''phone call'', ''broken display'' etc...
    • Simple +/- buttons in each line so you can press that after each phone call, email....
  • Every day service agent opens new daily form from url
    • data is collected to managers sheet on daily basis
  • We can count why our customers are contacting us and how many contacts each service agent had last week

 

Is this possible with some tool?
