Found that if you move a "step" from the bottom to the top, it won't refresh the list. So it will look like duplicates are created. However if you switch tasks, then the list is correctly refreshed.

Or you can also raise it through the To-Do mobile app by going to menu > help and feedback and selecting contact us in the top right of the screen.
