Visitor Form for requesting a visit and Approval by company user

Hi,

We would like to create a Visitor Form which will be filled by someone outside of the company.

This will have multiple questions and also the contact person and email of contact person.

Once the Visit Request is submitted, the contact person should get notified and contact person should be able to Approve or Deny the visit. Once Approval or Denial is Summited, the person who is visiting should get notified weather the visit request is approved or denied.

Is this possible? Does anybody have example of this functionality?

@shashipatra this isn't possible using only Microsoft Forms, you will need to start learning Microsoft Power Automate, another of the Office 365 apps. And it will also involve a SharePoiont list which will store all the visitor requests and the approval status etc. I'll post screenshots of a flow in Power Automate to manage this as soon as possible.
