Unknown access to a MS Form

A user in my org has created a form and circulated the collect responses link to required participants.  Before circulating the form they shorten the URL in MS Forms.  This created a duplicate shortened URL of a form created last year (both forms have the same shorten URL).  These forms are not linked.  This meant a user who received a link to the form created last year was able to access and view responses to the new form that has been created this year.  Any idea how this may have happened?  Can you remove access to a form after a link has been shared?

In this case, it is best to delete these two forms and create a new one - the correct one - only this is how you can remove access to the link.
