Several students were completing a quiz through Forms and when they clicked Submit it didn't record their answers but returned them to a blank Form. The previous answers were not collected at all. This was being done through Take A Test. The problem occurred over the course of 2 different classes and 2 different Forms for each. It happened to 3 out of 44 students. The other 41 students' worked as they should.
