Respondent Name & Email address are not getting stored in Dynamics

Dear Friends,

 

Whenever I submit Forms Pro without ctx passed in url, Respondent Name & Email Address are getting saved.

 

But whenever I submit Forms Pro with ctx passed in url, Respondent Name & Email Address are not getting saved in Dynamics.

 

Please help on this..
