@reginaj1985 Two things to check: 1) did you set the settings of the form to "One response per person"?; 2) You'll need to ask the responder if they had 2 tabs open with the form in each one. It has been reported that in this scenario it is possible for someone to submit more than one response even if the "One response per person" has been set.
