Questions Missing

I have setup a Quiz with 10 questions - The first two are a choice, next two rating and the 6 remaining questions a text response. All questions require a response.

All questions are visible when developing the quiz. When I go to preview, only the first two questions are there. I have tried on different browsers and there is no branching.

Any help would be appreciated.

If it helps anybody else - I added a branch (that wasnt required) which solved the issue.

I guess you enabled branching option in second question and under branching option you have selected the option "end of the form". That's why, nothing is visible after second question. Choose option "next" Under branching. This may resolve your problem.
