MS Forms embedded in Sharepoint Online have order of questions changed

I have created Set of questions / survey in MS Forms. The same Forms has been added into a Sharepoint Online Page. 

After publishing the page, the sequence of questions displayed in MS Forms within Sharepoint Online is changed.

For e.g. 

MS Forms Questions

1. What is your name
2. Your employee ID
3. Your Phone Number
4. Your work location
5. You concerns
6. Your Feedback Rating

and when the same form embedded within Sharepoint Online is launched in new browser

1. What is your work location
2. Your Feedback Rating
3. What is your Name
4. Your COncerns
5. Your employee ID
6. Your Phone Number
