Looking for multiple options and long description together in Forms

Hi, I am exploring MS Forms to take internal stakeholder survey with 5 different grades i.e. Excellent to Poor. At the same time, I like to know the brief description what the reasons for that grade. If user can fill option and small description. Unfortunately, I couldn't successful in getting this option. Any suggestions/

@EswaraS there is no option to do what you want. As a workaround you could use branching if the user had, for example, selected marginal or poor and take them to a new question asking them why they made their selection.
