Inserting Pre-Selected Data Into Form

I have a pre-existing internal web app which gets email addresses & a vendor name.

I would like it to trigger Forms to send a survey invite to those email addresses w/the vendor name already filled in. Is that possible? And if it is, how would I go about it? Thanks.

@emilypollard - are you using regular Forms or Forms Pro? With Forms Pro you could potentially use it combined with Power Automate to send an email out to the vendor and populate their name in the survey using variables. Something like this might help to give you some ideas:

It really depends what the other app is and how that data is stored.
