How to design a form with data based on SQL Server?

Can we create a Microsoft form with data based on SQL Server? If yes please let me know.

@skumar it's not possible as Forms has no ability to look up data from anywhere.

@skumar you could build it in Power Apps which can use SQL Server as a data source.
