How to allow the user to add text boxes in microsoft forms ?

I would like a create a form where the user can fill in their skill set/ competency and rate themself . I would also like the user to add any number of skill sets with an option " Add new" as shown in the attachment. Is it possible in Microsoft forms?

@ANUSHA1091 it's not possible in Forms to have the user enter likert statements in the way you have shown. It would need a form built with Power Apps instead.
