How do I create a Likert style layout that takes text inputs

I'm creating a question that asks for recommendations. I want the user to be able to type their recommendations and use the radio button to categorize the recommendation as "requirement" or "nice to have." Is this something we can do in Microsoft forms?

@kbrown108 it's not possible in the same question, you would need a question for their recommendation and then a separate likert question for the "must have", "should have" "could have" etc. If you had several of these you could use brancing to make it look a bit cleaner.
