How do I capture yearly leave details using MS online form?

I am looking for options to capture leave details for special leaves where the answers should be in the below format:

Start Date, End Date, Duration

We should also have the option to enter multiple events of leaves. E.g. below:

 

From Date

To Date

Half Day(0.5)/Full Day(1)

01/08/2018

01/09/2018

1

02/12/2018

02/12/2018

0.5

02/19/2018

02/22/2018

Assuming your wanting the person inputting to be able to see their entries etc.? If so, a SharePoint list is your better option than forms. And if you need anything more advanced you can customize those list forms with PowerApps.
