Forms Pro Title Section displays text at top of each section

I built a form survey form in Forms Pro with an image and the title in the title box.  There are two other text boxes in the title section.  When I first built the survey I had several explanatory paragraphs in the third text box.  This text only appeared on the first page.  All other pages only displayed the image and survey title from the first line.

Now, any text entered in the 2nd or 3rd box of the title section appear on every page of the survey.  What happened?

Could you provide a screenshot of what you are seeing? In the title section, I only see an option for the title, and an image, then the description. Where are you getting the third text box that you refer to?
