Forms in a PowerPoint Kiosk Presentation

I am creating a presentation to be used at kiosks in a trade show booth.

I would like to add a small form so that users can enter their contact information to get more info.

I am using Office 365 (ProPlus Ver 1908).  I used Microsoft Forms to create the form and added it to my PowerPoint. My problem is getting the user back to the presentation after filling out the form. The form takes over the slide - and I can't hide it through animation or force it another slide. Any ideas of how I can return the user to the presentation, in presentation mode after filling out a form?

Follow up question - If the same presentation with the same form is used at multiple kiosks, will the data be stored in one place or are the responses stored on the local machines?
