Formatting Date Picker to 12 months from today's date

Hi team

I hope you are all well. I am creating a sickness absence referral form for my employer. We only consider sickness episodes on a rolling 12 month period. 

Is there a way of formatting the date picker so that it will only ever allow the user to select dates that cover the previous 12 months from today's date (this will obviously change on a daily basis!)

It is likely that I will move this onto a PowerApp, however at present I want to ensure I can make this foolproof on Forms.

@JonFromHR Forms is a very simple app and there is no way to restrict the calendar like you describe. It would have to be done in Power Apps.
